At the end of each school year, Private AEDY Providers should complete the following items:

Review (and submit, if appropriate) your completed Private Provider AEDY Program application. Submission of a new application is only needed for AEDY Programs approaching their application expiration date. Not sure? Check the expiration date of your program's application on the dashboard of the Leader Services system (get help logging into IMS).

Frequently Asked Questions
Q: Should we wait for the school year to end before completing these tasks?
A: No - these items must be completed prior to the listed deadlines in the checklist. You do not need to wait to begin work nor to turn them in.
Q: If a student referral is marked as closed and the student is being referred to AEDY again, can the sending LEA re-open the closed referral?
A: No. A new referral must be completed every time a student is referred to an AEDY program. This is inclusive of secondary incidents. See a description of what to do for secondary incident placements here.
