Facility Review
A facility review will be conducted as part of the cyclical monitoring process. Facility reviews can take place virtually or in-person. Your monitoring chair will determine the format for the review.
Required Uploads and Steps
Prior to the facility review:
Prepare a copy of the program's floor plan and upload it to FSA 1 (this file will be deleted from Leader Services following the monitoring)
During the facility review:
Provide a tour of each room in the facility, starting with where and how students enter the building
Include any space which AEDY students may access, including classrooms, hallways, cafeteria, gym, counseling spaces, etc.
Answer questions about the physical space and its use
Highlight safety protocols (e.g. fire extinguishers and systems, emergency exits, etc.)
After the facility review:
Provide responses to any follow-up questions from PDE

Q: When will the facility review be scheduled?
A: The facility review is typically scheduled prior to the week of monitoring and must be conducted at a time where students are not in the building.
Q: What will be looked for during the facility review?
A: The facility review will cover all areas accessed by AEDY students, including entrances and exits, classrooms, hallways, counseling spaces, the cafeteria, the gym, and any additional spaces the students use. We will look for safety and security, including search protocols, item storage, and separation of AEDY students from other on-site programs; provided student services, such as accessibility, private counseling spaces, and protected student records; food service, including provision of food and location where students eat; and fire safety, including emergency exits and fire extinguishers.
Q: Who will be present during the facility review?
A: At least one program staff member who can answer questions about daily procedures within the space should attend.
