Required Uploads
Upload program-specific restraint policy which meets the requirements under Chapter 14 and Chapter 711
Upload program-specific procedures for restraint, inclusive of notice procedures
Upload restraint log for all students - includes all of the required reporting components for the Bureau of Special Education - for every child seated in the program, dated from the beginning of the longest-seated student's time in placement (in the AEDY program), inclusive of previous school years.
Under student records, upload all injury reports related to restraint for every child in placement from the beginning of their time in placement, inclusive of previous school years.
For records of students who are no longer seated but whose restraint records fall under the timeline, upload under student records and alert the monitoring chair of the students' information.

Frequently Asked Questions
Q: What should we upload if there have been no restraints?
A: If no student currently in placement has been restrained during their AEDY placement, upload a blank template of the document(s) used for restraint reporting.
Q: Should the restraint log cover the last 3 months?
A: No, the log should always be kept current at the AEDY program. Provide the log dated from the student seated the longest. For example, if Sarah has been in placement for a year, the log would span the year and include all students who were restrained during that year, even if they are no longer seated.
